Critical Appraisal of Individual Studies

In-vivo studies have shown that growth hormone enhances transmucosal transport of water, electrolytes, and nutrients. Endogenous growth hormone is responsible for stimulating normal skeletal, connective tissue, muscle, and organ growth in children and adolescents. Somatropin binds to growth hormone (GH) receptors and produces a variety of physiologic effects that can be classified as being direct or indirect. These effects oppose the action of insulin on fat and carbohydrate metabolism and are potentiated by glucocorticoids.

Missed Dose

In terms of stakeholder involvement, both guidelines12,13 clearly defined target users and the development groups, but did not report whether the views and preferences of the patients were sought. The methodology for the development of both guidelines12,13 were robust. Both guidelines12,13 clearly reported methods for evidence collection, criteria for selection, and methods for evidence synthesis. Overall, both guidelines12,13 were robust in terms of scope and development, rigour of development, and clarity of presentation. RhGH replacement therapy should be initiated at low dosages. The amount of medicine that you take depends on the strength of the medicine. There are no adequate studies in women for determining infant risk when using this medication during breastfeeding. Do not use Sogroya® if:

The beneficial effects of somatropin (growth hormone [GH] replacement therapy) in adults are now established. Long-term somatropin administration in GH-deficient adults improves body composition, muscle strength, quality of life, bone mass and density, and lipoprotein pattern. The extent to which somatropin therapy can also reduce cardiovascular morbidity and mortality in GH-deficient adults remains to be determined. Thorough long-term monitoring of glucose metabolism, cardiovascular measurements, and underlying pituitary disease, is, however, mandatory. The PES guideline provides recommendations for the clinical management of children and adolescents with GHD. The guideline begins with a recommendation on the use of GH to treat GHD.

Additionally, cerebral hypertension may be more common in Prader-Willi syndrome patients. When used to promote growth in pediatric patients with epiphyseal closure, somatropin is not recommended. No potentially relevant publications were retrieved from the grey literature search for full-text review. Of the potentially relevant articles, 16 publications were excluded for various reasons, and 2 publications met the inclusion criteria and were included in this report. Rare (possibly fatal) lung/breathing problems may be caused by this medication in children with Prader-Willi syndrome. Those at higher risk include males, severely overweight children, or those with serious lung/breathing problems ( such as sleep apnea, lung infections, lung disease). Children should be checked for certain breathing problems (upper airway obstruction) before and during treatment.

Patients using somatropin may experience decreased blood cortisol levels and/or the masking of central (secondary) adrenal insufficiency if they have or are at risk for pituitary hormone shortages. Following the start of somatropin therapy, patients receiving glucocorticoid replacement therapy for previously identified adrenal insufficiency might need to increase their maintenance or stress dosages. The safety of continuing somatropin therapy in patients receiving replacement doses for situations for which the medication has been approved has not been established. Therefore, in patients with acute critical conditions, the potential benefit of continued somatropin therapy should be weighed against the potential risk. Somatropin is also not recommended for use in children with Prader-Willi syndrome and respiratory insufficiency due to cases of mortality (see Prader-Willi discussion). Check your blood sugar regularly as directed and share the results with your doctor. Tell your doctor right away if you have symptoms of high blood sugar such as increased thirst/urination. Your doctor may need to adjust your diabetes medication, exercise program, or diet. If this medicine is used for weight loss/muscle wasting, it may take up to 2 weeks to notice the effects of the drug. Do not use more of this medication than prescribed or use it more often since the risk of side effects will be increased. Inject this medication under the skin as directed by your doctor.

Serum IGF-1 levels should be used to monitor response to changes in GH dose. GH treatment is continued until the growth rate falls below 2.5 cm/year. Patients should be aware of the potential short-term and long-term adverse effects of GH treatment. Re-evaluate the somatotropic axis in children diagnosed with GHD during the transition period (i.e., from pediatric to adult care).
